Here is a list of services we may have provided to you via our online order form found at www.evilsizor.co. 

• BOC-3:  Is required for ALL MC/FF/MX accounts filed with the FMCSA/USDOT.  Terms of service may be found here:  https://www.evilsizor.co/boc-3-terms.html
  
• UCR:  Required for ALL INTERSTATE carriers/brokers who has a USDOT number.  INTRAstate carriers are NOT required to file the UCR.  The UCR fees are not refundable.
• MCS-150:  A biennial update is required.  This may be filed either online with your DOT PIN or via an MCS-150 paper copy. www.fmcsa.dot.gov/registration/updating-your-registration
• Reinstatement:  Please note it may take a minimum of 4 days to reactivate your authority provided your insurance has posted to the FMCSA site.  Lack of insurance is the reason accounts are not activated.  The $80 filing fee is is NOT refundable once filed with the FMCSA.
